Here is a nice sample EUL for your software, it is a sample only and it is always suggested to have a lawyer review any license for validity. It cost me less than $200 to have a non-friend lawyer (I have a few lawyers that are friends, but none have software experience :)) with software experience to build me one based on a sample similar to this, I would assume you would pay only about an hour's retainer to have one review this if you were to put in your information:

I referenced a volume in an earlier posting that contains a wealth of information about licensing and other legal matters pertaining to software. The book is "The IT/Digital Legal Companion: A Comprehensive Business Guide to Software, Internet, and IP Law Includes Contract and Web Forms"
By: Gene K. Landy; Amy J. Mastrobattista. The authors are lawyers with a Boston intellectual property law firm and contains samples of licensing agreements. I accessed it through Safari, O[Reilly publishers on line library. The authors give full permission to use the material in their book.
